Supportive Housing Services

FCS offers supportive housing services to individuals experiencing barriers to housing stability, such as homelessness or unstable living conditions. These services include:

  • Housing Assessments: Evaluating individual housing needs and preferences.
  • Resource Identification: Connecting participants with suitable housing options.
  • Lease Support: Assisting with understanding and obtaining lease agreements.
  • Independent Living Skills Development: Providing training to manage daily living activities.
  • Landlord Relations: Facilitating communication and relationships with landlords.
  • Crisis Management: Offering support during housing-related crises.

To qualify for supportive housing services, individuals must:

  • Be at least 18 years old.
  • Be enrolled in Medicaid (Apple Health).
  • Meet specific risk criteria, such as chronic homelessness or dependence on institutional care.

Supported Employment Services

For those facing obstacles in obtaining or maintaining employment due to physical, behavioral, or long-term health issues, FCS provides supported employment services. These services encompass:

  • Vocational Assessments: Identifying individual skills and employment goals.
  • Employment Planning: Developing personalized employment plans.
  • Job Placement and Development: Assisting with finding and securing suitable employment.
  • Job Coaching: Providing on-the-job support and training.

Eligible participants for supported employment services must:

  • Be at least 16 years old.
